Items you will need to complete your  Registration and the Individual Zoom Assessment:

Thank You for working with MSDC Staff to find out what is needed to purchase your home and find Down Payment Assistance in the community    

 

In order to be registered for the seminars. 

​

Please complete the Documents below:  

1.  MSDC Homebuyer Intake Form Package 

2.  MSDC Disclosure Form

3.  MSDC Budget Form

4.  HT Third Federal Disclosure Form

5.  HT 2024 Manual     

6.  MSDC Homework Forms

Please also Provide:

1.  Copy of each attached document listed above:

2.  Copy of the front only of your Driver Licenses of everyone that will be on the loan

3.  Copy of the latest 30 days of income verification for each person on the loan

4.  Copy of your BANK Activity for the past two months.  (Please mark out your banking account number on each page of the Activity Statement)  Be ready to discuss information on where money will be coming from for closing.  

5.  Copy of HT HomeToday Worksheets for 4 weeks of tracking your spending worksheets

After we receive these signed documents, you can call in to schedule to meet one-on-one with a HUD Approved Housing Counselor to complete your individual counseling.  

​

MSDC will need these documents listed below to prepare for your scheduled assessment in our office 24 hour priors to the scheduled zoom appointment.  If we do not receive the documents 24 hours prior to the appointment, your counselor will have to cancel the appointment and reschedule after we have completed the intake process.
